INFORMATION WE NEED:

NAME, ADDRESS AND DATE OF BIRTH OF PERSON MAKING THE WILL;

NAME AND ADDRESS OF AT LEAST ONE PERSON TO ACT AS THE EXECUTOR OF THE WILL;

NAME AND ADDRESS OF THE BENEFICIARIES AND PERCENTAGE OF ESTATE EACH WILL RECEIVE; AND

WHAT WILL HAPPEN TO ANY RESIDUAL PROPERTY OF YOUR ESTATE.
